It varies. Not too different from what you describe below, but I
usually have several top level folders for different functional areas
with the components grouped into them.

I was taking ERExtensions as the Wonder style: top level grouping
full of a ton of stuff that is hard to find stuff in if you don't
know the name. But that is a grumble for a different list. :-)
