In my experience this type of problem is usually caused by:
a) an MTU mismatch between ISP and customer (usually accompanied by B)
b) ICMP has been blocked or isn't handled by NAT preventing Path MTU
Discovery (PMTUD) from working
c) a transparent proxy cache is in use and has cached bad data
